The facility offered 120 beds, surpassing the county average by 39 beds during the first quarter. On average, it provided care to 102.5 residents each day throughout the period.
CMS awarded Haralson NSG & Rehab Center an overall rating of 1 out of 5, which was lower than Georgia’s state nursing home average rating of 2.7.
Within the same period, Countryside Post Acute was the second largest with 62 beds, followed by Buchanan Healthcare Center with 60 beds.
As of the first quarter of 2026, Haralson NSG & Rehab Center had incurred $8,788 in a single fine and was subject to two penalties.

But when you need that type of care, there are limited options.”
